So, how do you put this case to gether?

What a quistion!
If ur doing ur clerk-ship you must have been asked this quistion nearly evry time u present a case or at least you heard many at clinical seasions.
In fact bieng able to make a correct diagnosis is one of the key aspect of evry medical student's and jonior doctor's clinical training. As medical student it is'nt about reaching the correct diagnosis it is really about how you develop the right the process of thinking because as long as you know how and what are the steps of reaching and handelling a case you can do the rest at your own .BUT FIRST we need to develop our thinking process .that needs lots of pts interveiwings, and co-aperative guide or at leasr a faithful fellow .

Now much of the clinical teaching and examinations in the new curriculums is problem –based.medical students will, relys on symptoms (Sx) or signs to elicit a diagnosis.

thus to meet any kind of clinical condition you need to
1. work out a good history of the case ( the symptoms)
2. complete physical examination to pic up the signs
3. then according the clinical picture you came with from step 1&2
decide which condition or donditions (diffrential diagnosis) that best fit ut case. In many cases you only need to think simple and to come with the answer that directly STANDS TO A REASON & that don't need over-thinking. let the rare answers at the end of you list unless you think that ur what you have in urcase is really suggestive.
4. also a medical student needs to know which basic clinical invetigations are approciate and which special investigations will nececcerary to characterise the problem. By this step the list of the D/D should be menimized or you even may reached to the final diagnosis.

We may all now these things but we really need to practice it more but in the right technique & approach . reading also helps.
5.Moreover I think this issue makes a wealthy area for us in our clinical yrs forum under a separate section we can call it medical recordsor clinical cases or whatever appropiate where we can
 presents cases summeries to learn from , or to disscus
 or if any member confront a diffecult case he or she may would like to display it there to other members so we can look for it read about it and find a solutions for it
 or recording any intersting cases by which well have a rich library for all our forum visitors to deepen their knowllage
So in summery:
Evry practitioner needs to develop the right skills by his own efforts to be able to deal with the clinical issues facing him or her. this may needs
• studying hard
• summer training,
• faithful and coaperative tutor
• group discussions
• reading book that deal with problem-based clinical appraoch for example
**Diffrential diagnosis by andrew T.raftery & Eric Lim
**the history & physical examinations casebook

and let's all remember that lrearnig is forever &have a vry great & successful year
