Introduction
Gastric cancer still occupies a leading cause of cancer death. how to find an early detection index and prognostic factors are mandatory in cancer prevention.(1)
The coexistence of gastric cancer and type III intestinal metaplasia is increased in frequency especially in the Intestinal type gastric cancer.(2)
Intestinal metaplasia is a well-established premalignant condition of the stomach that is characterized by mucin carbohydrate modifications.(3) The normal gastric epithelium typically produces'a neutral mucin, which is converted in cases with intestinal metaplasia, to
an acidic type of mucin, which is either a cyalomusin or a sulfamucin.(2)
The progression from a normal gastric mucosa through chronic gastritis , development of intestinal metaplasia to gastric cancer has been studied .It is this intestinal metaplasia that is thought to be a direct precursor lesion to the development of dysplasia and in turn , to the development of carcinoma . This happens with h.pylori infection, gastric atrophy , hyperplastic polyps and with healing ulcers .(2,4)
The issue of whether or not complete (typel) or incomplete metaplasia (type II&III) is the major precursor lesion to gastric cancer is controversial.(3)The most important determinant is not only the type , but the extent of metaplasia present .(2)
A correlation between change of mucopolysaccharide type , status of Helicobacter pylori and over-expression of P-53 was noted in carcinomas with intestinal metaplasia.(1) Also there are a number of some enzymatic changes and molecular alterations that can be seen in areas of intestinal metaplasia beside mucin changes , including alkaline phosphatase change,p53 mutations , loss of E-cadherin expression ,and ras mutations.(2)
PAS/Alcian blue (PAS/AB), high-iron diamine/Alcian blue (HID/ABand Auschin blue
PAS stain can be used to highlight presence , pattern and extent of intestinal metaplasia in gastric lesions. and in Barrett`s esophagus ,in order to delineate patients at risk for the subsequent development of cancer.(2,4,5)
Objectives of the Research:
1-To get an indication about the incidence of gastric cancer in Saudi patients
2-To study the incidence of premalignant lesions of the stomach in Saudi Patients.
3-To study mucin changes in areas of intestinal metaplasias in premalignant lesions of the stomach and correlate its type , extent and pattern with that of different types of gastric cancer
4-Training on tissue processing, routine hematoxylin and eosin staining and special mucin staining
Material:
The suggested number of samples for this research is about 50 samples of gastric cancer and premalignant lesions of the stomach ,either paraffin wax blocks or fresh samples during the period of this study (6 months)
Budget:
The suggested budget of this research for consumable materials is about 5000 SR
