Hello everybody..
this the OSCE Ex of medicine

Al noor ( male ) / Hera ( male ) / Al zahir ( female ) hospitals
6 station:
- history taking ( asthma / dyspnea )
- general Ex ( 2 station )
- CNS Ex ( motor & cranial nerves )
- Respiratpry Ex ( Oscult and pecuss )
- CVS Ex

Data interpretation:
6 station
- pic R. arthritis ( findings and diagnosis )
- pic sausage toes ( findings and DD )
- moon face ( findings and cause )
- ECG ( diagnosis: AF, rate,rthm and cuses )
- spirometry ( diagn: restrictive lung, write 2 causes of restrictve & obstructive lung disease )
- CXR ( diagn: pleural effusion, write the findings )

OVER ALL..it was easeir than expected.....!!!!

-the course time only 4 wks =16 days clinical teaching & 4 days for leactures.

-bad organisations in BST, leactures,

-what we nead to know though this course???

- bad organisations in clical exam & OSCE

these factors affect the outcome from this course

hello... ya really it was easy

why don't we unite and write some of the mcqs that we can remember

and i well start with what i remember

1-the first mcq was a case of a man with retrosternal chest pain lasting for 3 hours non radiating , the pt was sweating and nauseated
- the diagnosis ( MI - angina - unstable angina - )
- the enzyme u want to look for ( troponin - CK - LDH -AST- 1&2 )
-stupid mcq that says if the pt collapsed while u r talking to him what should u do ( start cpr - call for help - go and answer ur phone- give the pt food to eat)


2- a 14 year old boy tha presented to ER with fever and arthropathy following infection with group A streptococcai ?
-what is icluded in the major criteria of rhumatic fever( large joints arthropathy - small joints - peripheral joints -1& 2)?
- what is included in the minor criteria ( fever - arthritis - prolonged pr on the ECG- high ESR and CRP - all of the above)?
-what do u advice the pt ( avoid atrenious excersise - monthly pencillin injection >>>>>)?

3- female ,20 years old presented with a history of anemia and 2 episodes of jaundice , on exam she had splenomegaly and she was pale - on peripheral smear there was alot of spherocytes - her father have the same problem ?
-what is the best treatment for her ( splenectomy - immuneglobulins - blood tranfusion >>>>)?

4-what is the most common cause of colon obstruction( sdhesions - hernia - diverticulum - >>>)?

5- the most common cause of dysphagia in adult ( ca - webs - achalasia>>>)?

6 IBD is assosiated with all except ( fatty liver
- pyoderma gangrosum - uveitis - scleritis >>>>>)?

7- a uqu profeesor came to u for regular check u p he had a B.P=130/95 and the rest of vitals were slightly raised and his weight =120
- what do u tell the prof( u have HTN- come back within 2 years for check up - reduce ur weight >>)?

8- an mcq about broca's aphsia all are write except ( comprehension is preserved - conducting speech is defficult>>>>>>>)?

9- parkinsonism ( l-dopa and anticholinesterase and dopamine antagoinist is the best treatment - the side effect of l-dopa can be reuced by monitoring the dose - diagnosed by the regidity and gait and athirism - usuallu before age of 40 )?

10-status epilepticus( defined as continuous conylsions without loss of cosciousness - first drug to give is phenbarbitone >>>>>>>)/

11- girl presented with 2 episodes of hemoptysis:
- what history do u want to know( smoking - past asthma - relation to food >>>>)?
- what investigation that is unlikely to shows a diagnosis ( CXR - CT - spirometry >>>>)?

12-girl presented to ER with a lower limb weakness afetr a 2 weeks of diarrhea ,on examination there were waekness and areflexia , she was admitte to ICU and on the next day she developed upper limb weakness and weaknes of the face >>
- what is the possible diagnosis( MS - MG -POLYNEUROPATHY - GULLIAN.BAR SYNDROM)?
-what is the investigation u want to request( CT -MRI - lumbar puncture>>>)?

13-man presented with right side weakness(hemiplegia) he had only a sustained raised B.P , on doing CT..what is the most likely finding( infarction- hemorrhage - normal>>?)?

14-hepatoma( rarely metastasize - common in men - poor prognosis>>>)?

15- hypokalemia is initial charaterestic in ( acromegaly - cushing's - primary aldosteronism - hyperthyrodism )?

16 hypoglycemia( 50% of normal suger level - 70% - 80% - 90%)?

17-hypercalcemia ( high ca level with normal serum proteins - with highe serum protein - with low serum protein)??????????

18-one of the major features of antiphospholipid syndrom( all i remember is the one i select which is recurrent fetal loss)??i think there was also lavedo reticularis???


19-female presented with a sudden SOB AND CHEST PAIN , she was bedridden for the last month after a hystrectomy,
-what do u want to examine next( hands - arms - abdomen - legs)
-there were other mcq's about this case but i can't remeber them


20-pt with bacterial overgrowth in colon well shows normal( culture - xylose absorbtion test - shilling test that include injection of intrensic factors and vit.b12- fecal fat content>>>>)?

21- pt with acute asthma well show all except( >>>>>>>>>>>>>>>>>>)?

first : with all my resprct I think its dizaster to be in 5th
year and doesn't know what is required in medicine...!!!!!!!!
our tutors told us from last year 5th year course is 4th year(Hx and Ex) pluse investigation and some lines of Tx.

have u heared the word : MEDICINE IS READING EVERY THING ABOUT EVERY THING..

the other thing..I think u blame the organization in Al Zahir hospital ..I wanna tell u that the same problem we suffer in Al Noor and Hera hospital..but we should not sit and cry and blame the other..we should work hard and do more effort to pass and success..this the medical field (( NO SPONE FEEDIND ))
As all know Umm Al Qura is specialized in the BAD ORGANIZATION and COORDINATION..(( I think its cerebella disease..does it?????...........hhhhhhhhhhhhh

Investigations ==={ I mean data interpretation }
CSF, ascites fluid analysis , INR , ca correcting , urine analysis, how calculate CC by all formulas ,all types of radiological investigations … etc
All these investigations my ??? shell I know every things(details) of these in this course!!!!!!

*Haphazard (random) effort = you know everything at same moment you didn’t know anything
· if u don’t want mention the negative point to correct it and closed ur eyes about it and say everything is ok that mean u don’t need write about it .
