in this subject i'm asking you to stress on the defficulties that you are facing during your clinical years ..
even you, our colleagues in the 4th year , just tell us what are the problems that you are facing now
when you started your clinical coarse
what are the things that you were expecting , and what are the things that you found..

maybe we can help eachother and also the coming generations

Thank u Dr.wrood for starting this thread, & i think it is good idea to overcome our disadvantages wether from the collage or from us, I know we can not change every thing immediately but at least for the next coming generation ( one of our doctors said, to change for long time better than none ) & it is reseanably valid, really i do not want for them to repeate the same mistakes & trying to learn from the others.

I think one of the important diffecuties during the clinical years is loss of organization in some of the courses.

I`ll give u an example, when u r starting a new course, u have to know what u should know & what u should not know it at your level, & u have only your main book ( of course it is more than one ) and to do that u should have a schadual to follow it ( e.g Appendicitis: u should know the subtitiles i.e. embryology, incidence, eatiology, clinical features, invistigations ending by management ) and all that should be written in the contents of the schadual & so on.

The problem here is some of the departement is lacking to follow this appraoch, I am sure if they do that, this will make the student more comfortable & more understandable to his subject & avoid him to come at the night of the exam & he discovers that he has to study a lot of subjects he was thought it is not important but in facts it is.

So, please to the next generations, try to overcome this problem & do not be shy to tell your coordinator that i need a schadual to follw it in my studying.

what the problem i face it especially at the begining of clincal coarses .especially medicin.. that i wasnt know what i have to know ...
they told us history and pyysical examination ...
but they asked u more ...
i was so confused between the books .. which one i have to read and which one i will get benifit from it ... maybe there were some students who overcome this mashallah ...

Salam Doctors;

Thank you so much for opening such a subject coz right now that's what we really need for the coming 5 weeks.

I will go directly for what we faced during our 1st week in Medicine course.

1st, I want you all to immagine that we are 64 Male and 31 female students and Medicine depart. teachers are 4. that's right that some of the MOH doctors volunteer to be with us in BST but they aren't always free.

for my group, we started in KFH (sheshah hospt.) and it is really a ....... . no one with us, I can't exam all patients, not always can present you history to the tutor,,, etc. honestly at the 1st day I felt completly lost but I got used to it at the 2nd day. My Question for you doctors is:
Are there any advices for us get exper. within 5 weeks in such a setuation?? (N.B; we try to discuss our cases with each other a get our mistakes and correct them).

my 2nd Question is: what is the best book to follow in clinical exam that our teachers depend on in the final examinations??? I am reading from Talley the HX and phys. exam and then see Bate's the teqn. of the phys. exam and then try to find a case related to what i studied. but the problem is that they deffare in some points that make me totally confused. Also about kumar pocket, should we read it for our theory lectures or just depend on what we had studied in the lec. and what we are facing daily at hospital???

3rd Question (and the biggest problem 4 me): during taking the HX from the patient, and while i am getting the points from him/her, i take a looOoong time to rewrite it again and to arrange the paragraphs in order that makes me presint my case good to the doctor. my question is, do I have enough time to do that in our final clinical exam? or do we get faster in writing them with experience?

That's all what i got from our 1st week, I felt like baby lost in a toys store. we need your help if you don't mind Docs. and thanx again Dr.wrood2004 & Doctor MAJ for such a subject where we all can get benifit from our experiences.

thank u doc.lord for raising these important issues.....it's my pleasure to respond to ur Qs

ur concerns doctor lord here all r normal...esp that it's only ur 1st week...don't wory.......we all got through it and it past with peace......

about ur i1st Q.....I need to know how often u have bed side teaching ...... if once daily at least ......that meens u r Ok....dont wory....just keep on taking history & doing physical exam.....& present them to who ever available and and coaporative......
but if most of the days u dont have bed side....not even once
inform ur coarse cordinator please let know about the situation...

for the 2nd Q........the best book to study from is kumar....for the theoritical part .....also davidson is good ....just sticking to one of them ....is more than enough........

for the clinical part.....please study from taly it 's a gr8 book...... it never was deffecient of any information... ......u will have differernt opinion from each teacher.....but study from tally it well make u confedent of ur information....if u want to study from Barbra Bates just stdfy the CNS exanination........

for writing the Hx......u well get faster with the time ........not only that but u well be able to arrange ur Hx while u r taking it from the pt........and then u well be able to take & organize ur Hx in less than 20 min.......the trik is too keep on traing ur self by taking Hx as many as u can of ....1st try to be able to take a complete hx then try to to organize ur hx while taking it... then try to do that in 30 min min then in 20 min.....

finally dr.lord ur in from what u wrote i foud that ur and ur friends are at rhe right trak.....just keep on ....good luck

Thank you so much for the answers dr_perfecta and for your advices. I will try to do my best with my friends. there is something I waana notice; my confidence was better in my 3rd case that in my 1st case, I could ask more questions, do more things in the phys. exam. and talk better. I hope we can continue in that way. May god bless all of our students.

dr . lord i think dr.perfecta covered most of the points that you were coplaining of.... and Jazaha Allah kol 7'eer

for 1st q : i would like to advice you to concentrate on improving your skills in history taikng and physical examination.. that all what you need to do in your time at hopital ... try to get confidence by taking more Hx and doing more PhX ,,,,becouse this is mainly what the docotrs going to evaluate you in your final exam ..

so whenever you take Hx try to present it you and your friends in front of any volunteer dr from MOH that you feel he/she likes to teach you

2nd Q.. as dr.perfecta said ..

3rd Q..also dr.perfecta answered you ...
believe me you will be faster and better in it ... just give yourself time to practice it ..

PLZ if you NEED anything just ask ... and all of us here are ready to help you ...
we know that the stuff in our collage are few ... and we want to help them even here .. by helping you brothers and sisters ..

Plz brothers and sisters ..devide yourself to small groups and try to work as a team ... try to improve yourself by this way ..

try to dvide your days...
read one chapter in a day and come in the next day and discuss it with your friends ,, and raise the important points in it ..

also try to read the physical examinaion of any system in the body then try to apply it next day on any patient ..you and your friends and try to correct your mistakes as much as you can and improve yourself by this ...

Good luck D.lord 4 u & your colleague, I hope to be better & better in the next weeks too.

Mashallah Dr.wrood & Dr.perfecta have been said alot & INSHALLA f u follow what they said, u will be in the write place.

I want only to advice u that if u can read the important chapters in tally & barbra bates and do a notes for yourself & write the important points in each chapter systematically, this is will make it more easy to review it, to keep it with u once u r walking in the ward seeing the patients & also u can comment from thes notes in front of u on your freind when he present wether Hx or Ex & cretecise him f he is right or wrong. In addition to that, these notes will be with u in the whole your clinical years & u will need it continuously.

I think the important chapters for u is resp, git, cvs & cns, so try to concentrate on these 4 chapters.

I want just to reassure u & your collegue & what is happening to u especially u r in the first week is normal.
U will face some diffeculties in the beginning in thaking Hx & doing an Ex but beleive me all these difficulties will resolve with time, only keep on taking Hx & Ex & please repeate & repeate, that will make u as my collegue said confidince to present infront of your doctor.

I wanna ask ya about the clinical final exam, how is it gonna be, and what about the cases, are they chronic cases or new? and will be there any external examiners?? coz my group is 22 students and we will examed in 1 day.

hi Dr.Lord ..

clinical exam will be one long case for each student and it will equal 60% from the total marks.. the evaluation mainly will be for your history and physical exam .. dont surprise if they ask you about invistigation or some treatmet ...

try to expect any case .. they will bring anything they find ...
dont get surprise or confuse .. they will evaluate you only on your history and examination ..so dont get confuse ..just do what you are daily doing ..

ofcourse they will bring some external examinar ... maybe from alnour hospital or from the hospital that you will do your examination in it ..

the most important thing is : just try to be confident from yourself ..dont get confuse .. try to talk and work as a doctor during the examination ..if they ask you something that you dont know say ( I dont know ) dont try to say something rubbish ..

The most important note which i wanna mention here is:
The exam will be in the OSCI way >>> not a long case as i
examined last year.
*I took this information from Dr.Abull salam (face 2 face)

OSCI is easier than long case......
so don't be afraid and try to practice on OSCI exam way

I wiil try my best to put here the most applicable method in the history part spots of the OSCI exam >>> just wait....

in my opinion if the medical student didn't read more and practice more he well not get the full benifit....
because as you see there is no enough staff for that huge number of students

and as you said .. OSCE is much easier than long case exam and you can gain a good mark just be confident in it ...

yes you are right ..... the collage depend mostly on self learning way .
if you didnt read no body will tell you about some important details... but you can get it by attending round and session ..

" just try to lestin and understand carfully and if you didnt understand ask ... always ASK ASK ASK ....no shame from asking as you still student .. "
