For our colleagues and for the whole interns .
I would like in this topic to open a new discussion about the internship life .
Lessons To Be Learned From An On Call Duties
How to get a benefit from your on calls duty ? If you faced a problem in it , how will you manage ? Mistakes learned from it ? wrong habits against to follow it in your
future`s life …. Etc .
We can through our experiences and stories over here trying to correct our self if we did a mistake or at least to know how can we deal with it in future if we faced it again .
I`ll start talking , to be followed by others .

Four patients admitted to the ward , I`m the only intern on call that day with my resident , he asked me to see all of them , clerk , examine , write , extract blood , insert canula , insert catheter , follow up lab result , observe the patients …. And so on.
I was supposed to be ready also for preparing a case for presentation in the next day in the morning meeting .
Any way , each patient took from me about one hour or more , the time was at that moment 12.00 pm , and still the admission in progression in that day .
My resident was coming to me between a moment and another , ordering me to write the medication on the order sheath . Otherwise , he is setting , relaxed , comfortable in his room , drinking tea and watching TV .
Up till now , there is no problem , this is my work , and the more patient I touch , the more that I will get experience and learning .
The problem is , really funny , when the intern attach to some of the teams , the resident looks happy and lucky to have an intern in his team joining him . Why ? because if there is a lot of patient related to the same team , it will be seen now by the intern , if it is not all , will be the most .
And to clarify the item , i`m not against a hard work for interns , not at all , i`m with to be an active one , because this is the only chance lift to do mistakes and learn with an excuse .
But when you feel that , your experience and knowledge is not gaining with time , although you are trying to do your best , so , that means , there is something wrong .
Another one of my seniors , I was on call with her on another day , what was she do ? really was a nice one , I was lucky to join her that day , she was with me from the beginning of the case to the end , I was doing full clerking to the patient and handle him from A to Z except the medication which was ordered by my resident , and when I was preparing to my case presentation to the next day , she was informing , discussing , clarifying the things which could be asked during the morning meeting related to the case , and by this way , my work was more interesting for me because now I understand many things about it .
The point which I want to emphysize on is that , our seniors should understand that , we are in the hospitals not only for working in the whole day purely service , putting under that subject that because we are interns and we have to do every thing , no , our job is to be helped too by them , not to reduce the quantity issues of work rather than guiding , teaching , and respecting us as an intern who want to be a good physician in the future .
That what I have right now , I would like to listen also from my colleague what they have ??

well doctor MAJ......thank u for this gr8 topic.........u always choose things that matter most for us ..................
about hat u mentioned it very true fir me.......it just like u r taking about our situation in the hospital ian in..........I think now ti the way for mist of us ........they give u the chance to use ur cababilityto the greatest limit....but without the gaining much of new strategies to help to the rest of ur proffessinal life ..............any way it is so bad...........
I think for me and all those who expeirncing the same situarion ,we should demand an open dicussion between us and the academic affaire personnels in our hospitals to suggest some ideas about educational programs to help........or meeting with themjust to rais our concerns to thems..............
giong to another point u mentioned which is the senior rule..........i think what ever the situation never do somthing without dicussing it with ur senior........also for prenting a case first present it to him or her........
another thing important honesty is a simple and the best policy.......dont claim that u did somthing u didnt do it...........
finally do ur best ........for the sake of ur pt u r now a real physician......allways wach God in ur work.......
and as I read once ...( internship only last for 12 months..they cant hurt u but they cant stop ythe clock).:sad:

its sooooooooo nice to hear that from u
i thought that i am the only one facing that problem
really its nice to start ur job as a doctor doing many things
but what happens from the senior residints
is unaccepted some times
specially when they think by that way of
being happy to have an intern to do his job & stay watching TV without givivng his experience to the intern Or helping him
even though i have worked with some fantastic resedients
whom r happy to help u & let u learn & practice every thing
they do look for us to do the procedures
& we feel happy for that
really once u start practicing things for the 1st time & when they gave u the confedence
u will do it easly & will never forget it

Happy to hear these things from my colleague .
It means , we are facing same ideas , same problem , same people , and we can solve it together if we are insisting to do that in many different hospitals .

I agree with you that we should demand an open discussion between us and the academic affairs personele .
One day , my senior called me to see a child patient in the PICU , and I saw her and examined , the doctor told me the diagnosis , but when I go to read about the case , there was nothing correlated .
Then I went to the PICU again and I tried to discuss the problem with the doctor who was following her , she said , no , your finding is not valid !! I told her , I read about the case and I examined and nothing was correlated , she just was stopping me with different answers , even if i`m wrong , she has to explain why and give me a logic answer .
Few days later , the child was transferred to the ward , and I asked my senior to follow her myself .
Fortunately , my consultant in my team is a staff university , he came , he knows the case and I told him my impression , only me and him and no body else from our team was there at that time .
Initially , he reviewed the case , then he asked me to examine the patient again , I told him my finding , and he started to ask me several questions , after that we turned out our diagnosis to some thing else completely differed from the first one , and when I went to read about the new diagnosis again , it was just as we `ve been discussed , same finding with same history , and the management differed completely after that .
I was very happy to modulate this sort of discussion between me and my consultant , even if i`m wrong , I need some body to correct me rather than saying your talk is not valid !! :l:
Back to our point , that`s actually what we need really , an academic affairs personele helping us to make a decision confidently .
If every body trying to accept his mistakes even if it was from his student , the teaching and learning porpuses will be much much better to the joniour rather than considering ourself a senior and mature enough to say the write always .

I`ve been too faced those fantastic residents which u r talking about them , really they deserve to be consultants .
Giving u a great confidentialty to do a lot of things by yourself for the first time without even tremoring .

It's seem looooong time for last replay in this thread
mmmmm ........ 4 years !!!!

Till now I had 3 medical oncalls
but the best one I had it .... was with the chief resident
cuz he was with me from the moment they called us to ER till we admitted the Pts. and follow them
he discussed with me the correct steps to mannage the Pt. and told me the mistakes were done in ER
or even the wrong decision was taken by our specialist !!!
and before morning meeting ... we reviewed all cases that I'll present them
also he discussed with me the most imoprtant questions will be ask in the meeting !!!
Really ...... I was learnt alot from this oncall and from that great resident

If u had great, cooperative, updated resident "who really care to teach u the correct things"
in ur oncall .... u'll have fantastic oncall
