Actually for the pediatric part

Personally i didn`t did a form similar to it

But I think our staff doctors were giving us a sheath for it

There was also a hand card completed form

Given to us by one of them

And by the way

Is a good book for clinical practice in the hospital

During seeing the pediatric patients daily on the bedside
