هذه فورمة بسيطة عملناها
تنفعكم كثير إن شاء الله في أخذ التاريخ المرضي من مرضى الأمراض النفسية والعقلية

Salaam all,
I've received many emails appreciating this application. Some of them contain some mistakes, this is not a problem, every work can have some mistakes, Inshallah we will correct them later

This application can help you with the psychiatric patients but you can still go beyond it, so don't depend on it completely.

Finally I have to tell you that it is not seen by Dr.Ibrahim but we have used his lecture as a reference, but tell now we didn't see him to give him a copy. Inshallah we will try later.
