Hi everyone
Last saturday was our heama midterm exam .
To be honest , the exam wasn't easy , okay i wouldn't say hard also :ws: anyway !

the last Q was case ,
clearly , all the laboratory findings were indicating polycethemia .
* basophilia was noticed .
this diagnosis was part of the answer ,
But what was confusing is .......

All of us know that , symptoms & signs of polycethemia including :
Headache , Dyspnea , night sweat , plethoric appearance ,
70% got splenomegaly , hemorrhage " while platlets level is high but its function is defective " & PRURITUS " especially after a hot bath "

one point of the Q was What is the mechanism of Pruritus manifestation ?

I said it would be due to basophilia & small hemorrhages in the skin :?: , an other on said due to the hpertension :?:

I really do not know if any of u have specific answer ?
clinical years student would u help pliiz ?
i have searched at the book & also at the net and still no specific answer .
* i will not ask the doctor , waiting for your comments .

the pruritus is due to increased release of histamine from skin mast cells :MM: and basophils :D: which may relatively increased in polycythemia, and the presence of peptic ulcers in some polycythemic patients is again due to increased histamine release

it is written in Basic Pathology, 6th edition, page 379, so am not originating these informations :eye:

hema. is a ghost in third year :ws: but may ALLAH guide u all toward high grades

and thanx for mentioning the source dr zizo , otherwise , I may have called it " 6arf " , like what i do usualy at the exam ,

*looooool kidding ya :ws:

Now I know why 5-10% of polycethemic patients got peptic ulcer .
