I put this thread 4 asking HELP ... from our recently INTERNS!!

I would ask about MEDICINE in Al Zaher (KAAH) & Al Sheshah (KFH)

( Kindly ........ IN DETAILS)

1- Doctors

2- working time & oncalls

3- Cases

4- Responsibilities

5- Teaching activities

6- Availability of Sub-specialties teams

7- Staff's Cooperations with interns

8- Evaluations & Recommendations

cuz I knew those two hospitals are not the best in medicine :sad:

But .........

I'll try to enjoy my Medicine rota by filling the gap & come over defect's points !!!

HINT: I'm not asking help just from recent interns, but from any one having ideas about recent situation in those both hospitals :rolleyes:!!

Salam, I have good experience about Alshisha KFH, but dont know much about KAAH, so here is my opinion, it is a bit long but I hope it will help:

1- Doctors
4 doctors are really good, Dr Hani the head of department + Dr Farah but he may not stay for long time + Dr Abdulhammed Malak + Dr Hikmat although he is a bit quiet and you just try to ask about everything, the resident are also cooperative but you have to press on them if u are interesting

2- working time & oncalls
generally they are not very tough, many interns are not committed, so unless you are interesting, you can get absent and no body will really care, even in the oncalls, but if you are interesting usually you will take 4 to 5 oncall per month.

3- Cases
the variety of cases is really reasonable, you will cover all subspecialties of medicine including cardio and it covers a large area with almost all cases including oncology, although any complicated case is usually just stabilized and then transferred to other more specialized hospital like alnoor or national guard or KFSH

4- Responsibilities
usually this is determined by your resident or specialist, so it depends with which doctor you work, but generally in the first 1 or 2 days, you may just observe, but then most residents will give you some cases for daily follow up, so you will share the work with the resident, so basically your day will be like:
morning meeting from 8 to 8:30 or 9, it might be informative with some educational discussions but most of the time it is boring, generally it depends on the resident presenting, if he was boring then everybody will get bored and want to end the meeting as early as possible, but sometimes you may have a really good discussion with individual doctors, i advise you to try to present cases after oncalls (believe me resident will allow you and even will be happier that you want to take that headache from him) during your internship and try to learn from each case you present and ask during your presentation.
after morning meeting, residents will divide wards or cases among them, sometimes 2 or 3 residents cover male medical ward, and the other 2 or 3 cover female medical ward, usually intern will see cases with the specialist they belong to, so you go first read the file and know simple background about the case, then quickly read about it from a good book like washington manual, then go and see the case, take brief history about the new complaints and concerns, or if patinet is getting better, then examine the related systems quickly, then see if there is any investigations to follow up ask the nurse about it, and also see the doctor orders from the day before and try to make sure they are all done, finally write your notes in progress notes in SOAP system (subjective = history, objective = exam, assessment = impression if stable improving or unstable, plan = any new orders or just continue same treatment CST)
specialist usually do rounds at about 10 or 11 am, so just go with the specialist and present briefly about any new things in the case today, and at same time just write all orders or new plans in the file so you can comeback after rounds and make sure orders are done, whether a procedure or a new investigations or discharge summary, usually you will finish your work at 12 noon, most of the interns leave at this time, only interesting interns may stay up to 4 pm

5- Teaching activities
no regular teaching activities are running in KFH, but if you show a real interest, some specialists may just ask you to prepare a topic and present it in morning meeting or just for the interns or some residents

6- Availability of Sub-specialties teams
they have cardiologists, pulmonologists, nephrologist, and general medicine, and they have infectious disease doctors and wards حميات
they also have one rheumatologist who is very willing to teach Dr Sadek
but honestly not all of them are good enough and well qualified, just make sure you learn new things from GOOD doctors who are trusted, because sometimes many doctors are depending on experience but they dont have evidence so just be selective and be aware..

7- Staff's Cooperations with interns
generally there is no clear system and policy for the interns, unless you want to learn and you just press and insist on this, you can finish your rota without gaining much, but overall doctors are cooperative and even nurses

8- Evaluations & Recommendations
there is no single doctor with canadian board, so if you want recommendations from canadian certified doctors, you will not find anyone in Alshesha, but you can have some recommendation enough to get in Saudi board.

Generally, I think your medicine rota in Alshesha will be almost like a rest (FALLA), if you dont like medicine you will be happy, but if you want to get use of it, try to stuck to good specialists and daily discuss cases with them and try to ask about EVERYTHING (literally) and of course READ as much as you can about every case you see.
if you want to have time to study for Canadian exam or other exams, you will have it during this rota, you will see yourself having too much free time.

مره ممتاز الدكتور بس خليه يثق فيك من أول ما تجي
حبيت الشغل معاه == و كان لم يلف على مرضاه مرات يدورني بصراحة عمري ماشفت دكتور يدور ويسأل على طبيب امتياز بس كنت مهتم بمرضاه
و مرات لمن بيشوفك يأخذك معاه

طبعا اول شهر صعب شوي
بس بعدين عادي
بس كنت مع الدكنور ملك أستمع فأحاول اخلص شغلي قبل يسأل وامر معاه على المرضى

one more thing I want to add..
in Makkah, all the six Ministry of Health hospitals are almost the same (Alshisha, Alzaher, Ajyad, Alnoor, Heraa, Jarwal but there is no Medicine in it), so dont expect to see much difference between all these hospitals, I mentioned this before, but almost there is no good hospital for internal medicine in internship in Makkah, the best in Western region are King Fahad Armed Forces in Jeddah, National Guard, and King Faisal Specialist Hospital in Jeddah, and the reason is clear, because they dont follow the rules of Ministry of Health i.e. these are real teaching hospitals with organized teaching and training systems, while Ministry of Health hospitals are basically "service" hospitals and there is no system for teaching, only some individual efforts form good doctors

The problem for Internal Medicine is that it follows the "All or None" rule, so either you have a very clear & strong medical system, along with good & tough head of department, and lab and radiology systems, or otherwise the medical work will be like hell and never enjoyable, there is no grey zone, and it is almost impossible to find all these things in ministry of health hospitals..

so I guess the work in Alzaher will be almost the same, the difference will be in individual doctors, but the system is almost the same..

sorry if my reply is disappointing but this is the policy in ministry of health and I hope we can improve it soon in Makkah MOH hospitals
The good news is that we will have 3 new hospitals in Makkah soon which will be inshallah better than MOH hospitals: University hospital + King Abdulla Medical City + Security Forces Hospital, and I hope these will change the medical practice in Makkah to be as good as it is in Jeddah non-MOH hospitals (NGH, KFSH, KFAFH).
