You've been called to see a newborn after difficult delivery

Upon inspection, you noticed that he's having respiratory distress in addition to this lesion

What's the most likely cause for respiratory distress in this newborn?!



This newborn has Brachial plexus injury to C3, C4 & C5 (Erb- Ducchenne palsy) secondary to difficult delivery..
with the involvement of phrenic nerve (which has similar nerve roots)
it will result in diaphragmatic weakness that manifests as respiratory distress.


Erb-Ducchenne palsy, is a benign condition, usually results from difficult delivery. The usual picture is one of painless adduction, internal rotation of the arm & pronation of the forearm.
Moro reflex is absent on the involved side & Hand grasp is intact ..

during history-taking, you noticed that the child has stridor






What should worry you ?!!!














Associated subglottic hemangioma ..

















Which hemangiomas are especially worrisome ?!



Multiple cutaneous hemangiomas: may be associated with visceral hemangimas (e.g., Liver)

Large hemangioma: may be associated with Congestive Heart Failure.

Midline spinal hemangioma: May be a marker for underlying spinal cord abnormality.

Hemangioma with thrombocytopenia (Kasabach-Merritt Syndrome): life threatening condition of rapidly enlarging vascular tumor. In which the platelets are sequestrated within & consume coagulation factors to form Thrombi. Giving risk of DIC (more with tufted angioma).

90-95% of these tumors will resolve spontaneously, it's important to avoid the temptation of plastic surgery, cryotherapy, radiation therapy, or sclerosing agents, which can hasten resolution but lead to likelihood of scarring.
